Ordering medication My Pharmacy

You have indicated that you would like to have your medication sent to your home. After the recipe by prescribed to us, you will receive an email from My Pharmacy containing an order number, A link to the health questionnaire and a link to ordering the medication. Not received a link yet? Check your spam box or junk mail folder in your mailbox.

STEP 1: Go to www.123contactform.com/form-2128127/medicatie-gegevens-formulier this address is in the mail (see Figure 1). Fill in your medical information on the online form (Figure 2). This information is necessary for responsible medication monitoring.

STEP 2: After submitting your medical information as indicated above, you will receive an email containing your MED code.

 

STEP 3: Click on the link to order the product, the link can be found in the first received mail from MijnApotheek (see Figure 1). Enter the MED code and the order number in the spaces provided (see Figure 3). You can now directly the product to order.

Please note that the number spent on prescribed medication courses is always 1.

Once you have added the medication to your shopping cart you can enter your address information and checkout. Medication ordered on weekdays before 2 p.m. will be shipped the same day.
